According to HKFA, the are the same person. Seems that after the movie Mr Canton & Mrs Rose, he switch from being an actor to a crew. Maybe he had suffer serious injury.

Tai San (2) is another actor like Tai San. Proof: "Beheaded 1000"

Tai San (3) is also not the same person. I've seen "Long Hot Summer" and Tai San is not in the movie. It is possible that (3) is identical with Tai San (2) but I'm not sure.
